We specialise in providing the support you need in your own home or community. Our services are available to any person over the age of 18, who is living with a mental health condition, learning difficulties or physical disability. We also support clients who are terminally ill.
Our aim is to provide the support you need, when you need it, assisting you to remain in your home. This support may be short term due to illness or following a hospital admission to improve your independence. It could be longer term to help you maintain skills or support you with longer term illness or frailty.
We are happy to work within a multi-disciplinary team, e.g. district nurse, or other healthcare professionals to meet the needs of our clients.
Caring professional staff are on hand to offer a range of services which includes personal care, domestic assistance, community access and companionship. From a 30 minute welfare check to live in care, day or night or a 24hr block 7 days per week.
Care needs are individually assessed in partnership with our clients and, where appropriate, their family. The following is a list of the some of the support our care staff can provide:
